Parents Cooperative Preschool requires 5 forms to be filled out and signed by parents. The 4 that are required by The Department of Child and Family services in order to maintain our license are first. Please note that the first form must be signed by a physician. The fifth form is a Parents Cooperative Preschool general permission sheet that also needs to be on file. Please  read carefully. The emergency transportation part at the bottom must be signed without exception.
