Required State Forms for 2K, 3K and 4K

• immunization record 
• health history emergency care plan (This form must be completed and turned in before first day of school.)
• child care health report signed by health care provider
• child care enrollment form (This form must be completed and turned in before first day of school.)
• PCP permission form
• If you signed up to be a parent helper in the classroom, please follow the steps on2K and 3K NEW Parent Helper Training Info and fill out/turn in a 2023 background check form

2K and 3K Tuition Payment Type selection sheet (this form is required to designate how monthly tuition will be paid)

2K and 3K ACH Sign-Up Form :  This form is for families who would like monthly tuition payments taken out electronically or for families to update their ACH information.  ACH is free.

There is  a $10 annual fee for Cash/Check tuition payments.
